Enhanced Primary Care Plan:

A care plan is a structured plan created by your GP to help manage a chronic medical condition. It’s designed for patients who need a multidisciplinary approach – meaning care from a GP plus at least two other health or care providers. With a care plan, you may be eligible for a Medicare rebate on certain allied health services, making treatments more affordable.

mY AGED CARE

My Aged Care is a government funded initiative that offers a range of different support services for those over the age of 65. After assessment to deem eligibility, a range of services can be accessed such as help at home for domestic tasks, short term care options after a hospital stay or illness, or assessment and assistance for supported living arrangements. HOME CARE PACKAGES

Home Care Packages are a way for the Australian government to provide financial assistance to individuals over the age of 65 who need help with daily tasks and want to remain living at home. National Disability Insurance Scheme

The NDIS is a government initiative developed to provide allied health, occupational, transport and daily living support for people living with medically documented chronic health conditions. This includes a range of Neurologic conditions such as stroke, multiple sclerosis, motor neuron disease and Parkinson’s disease. To be eligible you must be under the age of 65 at the time of your application.
